How can I set up items to appear only on the first page of a report or only after the first page? This is for the logo and report title, among other things.

You should be able to accomplish what you want by choosing "Add Master Page" from the Master toolbar item. Each "master page" might be looked at as a "section" of a report--you might have, for instance, a title page, then a beefy middle, and then some kind of summary, each of which is actually a "master page" with its own layout.
