Through some "Monkey Work" and frustration I created:

This is based on a REPORT>END OF SHIFT and then I provide the information (redundantly) and much less detail than I need.

I was wondering if someone could assist me in a report that I could report actvities, notes and opportunities.. Right now I managed to "rig" strangely some form varibles to the report..

Help would highly be appreciated.
