We have just migrated our telephone system to a VOIP system. I need to create a directory in the following format....

CSV file must be in the format "lastname","firstname","displayname","phonenumber"
Or "lastname","firstname",,"phonenumber"

I know I can export from DayLite, but have never managed to get the files I needed, anyone out there that can help me please.

You need to first setup the view to show as a List. Then you choose to display only the columns you need and in the order you want them to be exported. Then you use the File:Export:Export visible columns and rows to File command and choose CSV format. Have you tried that? If you need any help on doing any of these steps, just ask.
